The term &#8220;anal stenosis&#8221; refers to an <strong>abnormal narrowing of the anal canal lumen<\/strong>, which is associated with <strong>defecatory difficulties of varying severity<\/strong>. These difficulties are often accompanied by pain, a sensation of incomplete evacuation, bleeding, and, in some cases, secondary incontinence due to prolonged straining. The clinical picture is typically dominated by a well-known <strong>pathophysiological triad<\/strong>: <strong>muscle spasm<\/strong>, <strong>pain<\/strong>, and <strong>defecatory dysfunction<\/strong>, which create a <strong>vicious cycle<\/strong> that can become self-perpetuating if not interrupted promptly. It is important to distinguish this condition from a simple functional narrowing of the anal canal, secondary to reactive muscle spasm caused by painful lesions such as common anal fissures.<br \/>\n&nbsp;<\/p>\n<h3><strong>Classification of anal stenosis<\/strong><\/h3>\n<p>The most common classifications are based on the severity and extent of the stenosis, as proposed by Liberman and Thorson in 2000. <strong>Severity<\/strong> is assessed based on the width of the anal opening observed during digital examination, while the <strong>level<\/strong> of the stenosis refers to its distance from the dentate line.<\/p>\n<p><u><strong>SEVERITY<\/strong><\/u><\/p>\n<ul>\n<li><strong>Lieve<\/strong>: si caratterizza per un restringimento che consente comunque l\u2019introduzione del dito indice lubrificato durante l\u2019esplorazione digitale o con l\u2019ausilio di un divaricatore. La compliance tissutale risulta ridotta, ma la manovra \u00e8 eseguibile senza necessit\u00e0 di dilatazioni forzate.<\/li>\n<li><strong>Moderata<\/strong>: il canale anale non consente l\u2019introduzione del dito indice lubrificato o del divaricatore se non tramite una dilatazione forzata, a indicare una compromissione significativa dell\u2019elasticit\u00e0 e del calibro del canale.<\/li>\n<li><strong>Severa<\/strong>: si configura come un restringimento marcato, in cui non \u00e8 possibile introdurre nemmeno il dito mignolo lubrificato senza ricorrere a manovre di dilatazione.<\/li>\n<\/ul>\n<p><u><strong>LEVELS<\/strong><\/u><\/p>\n<ul>\n<li><strong>Low<\/strong>: more than 0.5 cm distal to the dentate line.<\/li>\n<li><strong>Mid<\/strong>: 0.5 cm below or above the dentate line.<\/li>\n<li><strong>High<\/strong>: more than 0.5 cm proximal to the dentate line.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h3><strong>Etiology<\/strong><\/h3>\n<p>Several conditions are recognized as predisposing factors for the development of anal stenosis:<\/p>\n<ul>\n<li><strong>Congenital<\/strong> (very rare form, present from birth and predominantly diagnosed in pediatric age, often associated with anorectal malformations).<\/li>\n<li><strong>Acquired<\/strong> (secondary to external factors such as trauma, surgical procedures, radiation therapy, infections, or chronic inflammation).<\/li>\n<li><strong>Neoplastic<\/strong> (due to the presence of anal or rectal tumors causing lumen narrowing either by direct invasion or surrounding fibrotic reaction).<\/li>\n<li><strong>Inflammatory<\/strong> (associated with chronic inflammatory bowel diseases, such as Crohn\u2019s disease, which can lead to fibrosis and stenosis of the anal canal).<\/li>\n<li><strong>Spastic<\/strong> (caused by persistent muscle spasm of the anal canal, often related to chronic painful conditions such as fissures).<\/li>\n<li><strong>Anorectal stenosis from extrinsic compression<\/strong> (caused by adjacent pelvic masses or pathological processes that compress the anal canal externally, such as abscess collections, fibrosis, or pelvic tumors).<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<h3><strong>Diagnosis and treatment of anal stenosis<\/strong><\/h3>\n<p>The diagnosis of anal stenosis is primarily clinical and often evident upon inspection of the perianal area. <strong>Digital rectal examination<\/strong> is the key step: passage of the finger through the narrowing is often difficult or impossible, confirming the diagnostic suspicion.<\/p>\n<p>Treatment varies according to the severity of the stenosis and the underlying cause. In mild cases, conservative approaches such as <strong>gradual anal dilations<\/strong> combined with the <strong>use of laxatives<\/strong> to soften stool and reduce straining may be effective.
